What about Health Insurance and Long-Term Care?

During the last few years and largely in light of the Affordable Care Act, health insurance has changed dramatically.  Rates and deductibles have increased and in many cases, coverage has been reduced.  There are co-pays, maximum out-of-pocket, co-insurance, HMO's, PPO's and HSA's.  There's Fee-for-Service vs. Managed Care; Annual Open Enrollment and Qualifying Events to be eligible to buy a policy outside the "AOE" (Annual Open Enrollment).    WHEW !

Then there's Short-Term Care, Long-Term Care, Activities of Daily Living (ADL), non-cancellable, guaranteed renewable, coordination of benefits and COLA (cost-of-living adjustment -- not a beverage!).  So how do you navigate all of this, especially when it's changing so frequently?
